Evaluation of Homocysteine Level as a Risk Factor among Patients with Ischemic Stroke and Its Subtypes

چکیده مقاله:
Background: Epidemiological research has shown that increased total homocysteine (tHcy) levels are associated with an increased risk of thromboembolic disease; however, controversy still exists over which subtype of stroke is allied to hyperhomocysteinemia. This study aimed to investigate whether elevated tHcy is an independent risk factor for ischemic stroke and to compare tHcy levels in patients with ischemic stroke subtypes. Methods: We performed a case-control study, in which 171 ischemic stroke patients aged over 16 years and 86 age and sex-matched controls were eligible to participate and were enrolled from January 2009 to January 2010. The patients’ demographic data, traditional stroke risk factors, and the results of fasting tHcy, vitamin B12, and folate of serum were collected in the first 5 days after ischemic stroke. Stroke subtypes were classified according to the Trial of Org 10172 in Acute Stroke Treatment (TOAST) criteria. SPSS software (version 13) was used for the statistical analysis of the data, and a P value smaller than 0.05 was considered statistically significant. Results: The mean fasting Hcy levels was significantly higher in the cases (16.2 μmol/L, 95% CI: 14.8 to 17.5) than in the controls (13.5 μmol/L, 95% CI: 12.4 to 14.6) (P=0.013). The mean Hcy levels was elevated significantly in those with cardioembolic strokes compared with the controls (17.7 μmol/L, 95% CI: 14.8 to 20.5; P=0.010). The plasma Hcy level was associated with an adjusted odds ratio of 2.17 (95% CI: 1.24 to 3.79; P=0.004) for Hcy above 15 μmol/L concentration for all types of stroke. Conclusion: Our data showed that elevated serum Hcy is an independent risk factor for ischemic stroke and it has a strong association with cardioembolic subtype.

متن کاملEvaluation of plasma homocysteine level in ischemic stroke patients according to migraine history
Background: As yet migraine has been established as one of the risk factors for ischemic stroke. Some of the factors have been assessed for the explanation of this relation. Hyperhomocysteinemia is seen partly both in stroke and in migraine. This study was conducted to determine the mean plasma levels of homocysteine in ischemic stroke patients on the basis of migraine history. Methods: This cro...
متن کاملHomocysteine--risk factor for ischemic stroke?
The present study was undertaken to compare the homocysteine levels in patients of ischemic stroke with controls. Our study included 117 patients of ischemic stroke and 101 controls. The mean homocysteine levels in patients with ischemic stroke were 16.80 +/- 6.71 micromol/L while in controls it was 12.30 +/- 4.68 micromol/L, the difference being statistically significant (P < 0.01). The increa...
متن کاملInfection and risk of ischemic stroke: differences among stroke subtypes.
BACKGROUND AND PURPOSE Although prior studies have demonstrated that 25% to 35% of stroke patients have had a recent infection, the role of infection as a risk factor remains unclear. Our aim was to characterize the effect of infectious/inflammatory syndromes on stroke risk. METHODS Case-control and crossover analyses of 233 cases and 363 controls aged 21 to 89 years were performed. Cases wer...
متن کاملEffect of Lithium on Brain-derived Neurotrophic Factor (BDNF) Level in Patients with Ischemic Stroke: A Clinical Trial
Background and Objectives: Previous studies have indicated that lithium may increase the level of the brain-derived neurotrophic factor (BDNF), which in turn improves the recovery of patients with stroke. In this controlled trial we evaluated the effect of lithium on BDNF serum level in patients with ischemic stroke. Methods: In this randomized controlled...
متن کاملCerebral microbleeds as a risk factor for subsequent intracerebral hemorrhages among patients with acute ischemic stroke.
BACKGROUND AND PURPOSE Cerebral microbleeds (MBs) detected by gradient-echo MRI are considered evidence of advanced microangiopathy with potential for further bleeding. The goal of this study was to determine whether the presence of MBs is a risk factor for subsequent intracerebral hemorrhage among patients with acute ischemic stroke. METHODS We prospectively examined patients hospitalized wi...
